1. EVENT AND VOLUNTARY PARTICIPATION. I request permission for the named attendee to participate in the Striking a Nerve: The True Anatomy and Physiology seminar on November 21 and/or 22, 2026 (12:00–2:00 PM each day) at Burning Tree Mixed Martial Arts, 10210 South Dolfield Road, Suite C, Owings Mills, Maryland 21117, with guest instructor Dr. Frederick Weiss, MD, DPT. Day 1 addresses Kenpo; Day 2 addresses other martial arts. Instruction may include classroom explanations, movement demonstrations, supervised striking or grappling motions, partner drills, and anatomy-related activities. The attendee may decline or stop any hands-on activity.

2. KNOWN AND UNEXPECTED RISKS. Hands-on martial arts and movement demonstrations have inherent risks, including physical exertion, contact, loss of balance, falls, joint stress, bruising, muscle or ligament injury, strains, sprains, fractures, and—in rare cases—serious injury. Discussions of anatomy and technique are educational, not a guarantee of injury prevention or medical treatment. The attendee may participate in observation-only portions without attempting movement drills.

3. SAFETY AND MEDICAL JUDGMENT. The attendee agrees to follow instructor directions, exercise control during drills, avoid unsafe pressure or force, notify instructors of concerns, and stop if experiencing pain, lightheadedness, or discomfort. The attendee or guardian is responsible for deciding whether an activity is suitable and may seek personal medical advice before participation. This educational seminar does not establish a patient-provider relationship or replace individualized medical evaluation.

4. INHERENT RISK ACKNOWLEDGMENT. I understand and voluntarily accept the inherent risks of the activities the attendee chooses to perform. This is an acknowledgment of activity risks, not a waiver of claims for negligence or other wrongful acts of Burning Tree Mixed Martial Arts, the facility, its agents, or on-duty employees, or any other legal rights that cannot be waived under applicable law.

5. EMERGENCY ASSISTANCE. If urgent help is needed, I authorize event staff to contact emergency responders and provide basic first-aid assistance within their training. Treatment decisions belong to emergency services and licensed medical professionals. Medical costs and insurance responsibilities are determined under applicable law and coverage.

6. PARENT OR LEGAL GUARDIAN (MINORS). If the attendee is younger than 18, I confirm I am the attendee's parent or legal guardian, authorize participation in the selected event day(s), have reviewed the risks and safety rules, and will communicate the safety instructions to the minor. I may decline particular demonstrations or exercises for the minor.

7. NO MEDIA CONSENT. This form does not authorize use of the attendee's photograph, voice, name, or likeness in advertising. Any such permission requires a separate, optional authorization.

8. ACKNOWLEDGMENT AND SIGNATURE. I have read this entire acknowledgment, had an opportunity to ask questions, and understand that participation is voluntary. Adults sign only for themselves; parents or legal guardians sign for minors in their care. I intend my typed name to serve as my electronic signature on this document.
