United by Purpose — Participation Risk Acknowledgment & Consent

1. EVENT AND VOLUNTARY PARTICIPATION. I request permission for the named attendee to participate in the United by Purpose fundraising martial arts seminar on November 15, 2026 (2:00–4:00 PM) at Burning Tree Mixed Martial Arts, 10210 South Dolfield Road, Suite C, Owings Mills, Maryland 21117. Training may include demonstrations, warmups, supervised partner drills, striking motions, defensive movements, and other physical activity instructed by Master Mike Lambert, Master Lee Epperson, and assisting staff. Participation in any drill is voluntary; the attendee may decline or stop any drill.

2. KNOWN AND UNEXPECTED RISKS. Martial arts training involves inherent risks, which may include falls, collisions, contact with other participants or equipment, joint or muscle strain, sprains, bruises, cuts, fractures, head or neck injury, and—in rare cases—serious injury. Even careful instruction and protective measures cannot eliminate all inherent risks. I understand the activity may involve physical exertion and partner contact.

3. SAFETY AND MEDICAL JUDGMENT. The attendee agrees to listen to instructors, follow safety instructions, use controlled contact, report hazards and injuries immediately, and stop if experiencing pain, dizziness, discomfort, or uncertainty. The attendee should not perform activities beyond their capabilities. The attendee or guardian is responsible for determining whether participation is appropriate and may consult a healthcare professional before the event. No instructor is providing individualized medical advice.

4. INHERENT RISK ACKNOWLEDGMENT. I understand and voluntarily accept the inherent risks of the activities the attendee elects to perform. This paragraph acknowledges risks inherent in martial arts participation; it does not waive any legal claim for the negligence or other wrongful acts of Burning Tree Mixed Martial Arts, the facility, its agents, or on-duty employees, or any other rights that applicable law does not permit to be waived.

5. EMERGENCY ASSISTANCE. If the attendee needs urgent help, I authorize event staff to contact emergency responders and provide basic first-aid assistance within their training. Emergency services and medical professionals make their own treatment decisions. I understand that costs and coverage for medical care are determined under applicable law and insurance arrangements.

6. PARENT OR LEGAL GUARDIAN (MINORS). If the attendee is younger than 18, I confirm I am the attendee's parent or legal guardian, authorize the minor's participation, have reviewed the risks and safety rules on the minor's behalf, and will communicate these rules to the minor. I understand that I may decline particular activities for the minor.

7. NO MEDIA CONSENT. Signing this acknowledgment does not grant permission to use the attendee's image or name for marketing. Any photography or video permission will be requested separately.

8. ACKNOWLEDGMENT AND SIGNATURE. I have read this entire acknowledgment, had an opportunity to ask questions, and understand that participation is voluntary. If signing as an adult attendee, I sign only for myself. If signing for a minor, I confirm my authority as parent or legal guardian. I intend my typed signature to be my electronic signature for this document.
